How do I file a complaint?

Download the ADRA Complaint Form from "Complaints" tab on the website. Complete the form in its entirety including all details and information related to the complaint. You may use the pages provides in the form or attach a typed complaint along with the first page of form. Include as much information as possible; including the first, last names and contact information of all individuals involved. The completed and signed complaint forms should be mailed to the ADRA office to the attention of the Executive Director.
